  • Other acceptable forms of address proof include utility bills (electricity, water, or gas), bank account statements, credit card statements, post office passbook, property registration document, property tax assessment order, domicile certificate, address certificate signed by a MP, a MLA, an MLC, or a Gazetted Officer, certificate from the employer, or accommodation allotment letter issued by the Central or State Government.

Age Proof

  • Certificate of Birth, Passport, or Aadhaar Card are valid forms of age proof.

Note that the identity proof and address proof can be provided by any of the minor applicant’s parents or guardians and will be considered as the minor applicant's proof of identity and address.
